Designation of an agent of the provider who is authorized to receive communications and notices pursuant to this Section. d. Identification and justification of which FCC “shot clock” the proposed application must comply with. H. APPLICATION REVIEW: 1. GENERAL: The following provisions shall govern the issuance for new wireless communication facilities. a. If the wireless communication facility is a permitted use, a wireless application shall be applied for. This application will be used to review the location, lot size, height of the tower, stealth design for least intrusive design, and closure of a significant gap in service coverage as outlined in the providers communication plan. (1) Development Services Staff will review the application for completeness within seven (7) business days of submittal. (2) Applications which are not completed and fully comply with the requirements for the providers communication plan (Section G) will be deemed insufficient and the applicant will be notified in writing of the deficiencies. Applicant will need to resubmit a new complete application. (3) Application fee is $1,500. Applications submitted without the application fee will be determined to be incomplete and rejected. (4) Development Services Staff will review the application based on compliance with the zoning ordinance requirements for the specific district standards where the wireless communications facility will be located. (5) Staff will conduct a site inspection to determine if the design of the wireless communications facility is the least obtrusive method for design. (6) Staff will make a formal recommendation to the Development Services Director for approval (including approval with stipulations), or denial of the application based on staff’s findings that the proposed location and configuration does not meet the intent of the permitted use. (7) If the application is approved, a letter will be issued within 60 days of the date the application was deemed to be complete. (8) A letter of approval for a wireless communications facility shall be valid for 180 days from the date of approval. Building permits must be applied for within the 180 days. A single 180-day extension may be requested from the Development Services Director, subject to a $100 extension fee, provided no substantive changes are made to the original application. (9) If the application is denied based upon a finding that the proposed location and configuration are not the least intrusive means to fill a significant gap
